How do I upload medical records to ABHA?
Use an ABDM health record app: the ABHA app or Aarogya Setu 2.0.
- New records: at an ABDM-compliant hospital or lab, consent to link the records to your ABHA address. They then appear in your app.1
- Old paper records: upload scanned copies to the app's health locker. In Aarogya Setu 2.0 this runs through DigiLocker, which you set up once.1
- Sharing: no doctor sees your records unless you grant consent. You choose which records, and for how long.2
ABHA is free. Type your Aadhaar number or an OTP only on abha.abdm.gov.in, the ABHA app or the Aarogya Setu app. Many lookalike “ABHA card” websites exist. Naked Compound is not connected to the National Health Authority, and we never ask for your Aadhaar, OTP or health records.

Before you start
You need an ABHA profile. If you don't have one, see how to create an ABHA number. Then install a health record (PHR) app. The ABHA app and Aarogya Setu 2.0 are both run by the National Health Authority; other ABDM-approved PHR apps also exist.12
Log in with your mobile number, Aadhaar number, ABHA number or ABHA address.1
Get records from hospitals and labs
When you visit an ABDM-compliant hospital, clinic or lab, they can ask your consent to link the records they create to your ABHA address. Once linked, the records appear in your app automatically. You can also search for (“discover”) records from facilities you've visited and fetch them.1
The app shows several types of record. NHA lists eight in Aarogya Setu 2.0: diagnostic reports, discharge summaries, OP consultation notes, prescriptions, immunisation records, wellness records, health documents and invoices.1
Many hospitals have a QR code at the registration counter. Scanning it with the app (“Scan and Share”) shares your profile so you register faster, and some doctors' desks have a QR code for sharing records.1
Upload your old prescriptions and reports
- Open Aarogya Setu 2.0 and set up DigiLocker when the app asks. NHA says this is required to upload, store, view and share records in the health locker.1
- Scan or photograph each paper record clearly: flat, in good light, with the whole page in frame.
- Upload the file in the app's records or locker section and label it (for example “Prescription, Dr Rao, March 2025”).
The app can turn uploads into AI “Smart Reports”. NHA itself warns these may have limited accuracy, so always keep the original and check it.1
Our free prescription to digital record tool turns a printed or typed prescription into a clean PDF, with doses like “1-0-1” and “BD” in plain words. It runs entirely on your phone. Upload a photo of the original too: a typed copy is not a valid prescription.
Share records with a doctor, on your terms
Linking records to ABHA doesn't make them public. A doctor or hospital sends a consent request; you see it under notifications or the Consent tab, choose which records to share and for how long, and tap Grant.2
Consent is granular: you can share one report and keep the rest private. NHA's advice is to share everything relevant with your treating doctor so they have the full picture.3
Changed your mind? Revoke a consent from the Consent section of the app.2
Uploading records to ABHA: FAQ
Can I upload my old medical records to ABHA myself?
Yes. In Aarogya Setu 2.0 you can upload scanned copies through DigiLocker, which works as your health locker. Records created at ABDM-compliant hospitals and labs can be linked to your ABHA address with your consent.
Can a doctor see all my records once they are linked?
No. A doctor only sees the records you choose to share in response to their consent request, for the period you set. You can revoke consent later.
Which app should I use to upload records?
The ABHA app and Aarogya Setu 2.0 are run by the National Health Authority. Other ABDM-approved PHR apps exist too. Download them only from the official app stores.
Is a scanned or typed copy of a prescription valid at a pharmacy?
A scan is a copy of your record, not a new prescription. Pharmacies may need the original prescription from your doctor. Keep the originals.
Are AI Smart Reports accurate?
Not always. NHA says Smart Reports may have limited accuracy and advises checking the original records and asking a doctor before any clinical decision.
Is it safe to store health records in ABHA apps?
NHA says Aarogya Setu complies with India's Digital Personal Data Protection Act and ABDM's federated design, where records stay with the hospitals that created them until you consent to share. Use only official apps, and never share your OTP.
